You do not need to participate in ATVAM to become a member of Mudslingers OHV Club.  If you would like to join ATVAM you can submit this form along with the Mudslingers OHV Form and the club will submit the form to ATVAM.

Mudslingers OHV Club

With your membership you are releasing Mudslingers OHV Club of any liability at any Club activities. 

With your membership if you are participating or observing any ATV activity with Mudslingers OHV Club you agree that you are aware that ATV activities are hazardous and involve many risks and dangers and can result in serious personal injury or death.  You waive any and all claims that you may have or may have in the future against Mudslingers OHV Club and release any and all liability for any loss, damage, expense, and injury including death that you may suffer while participating or observing any activities with Mudslingers OHV Club. This also includes any personal injury or property damage.
